What the document holds

Joint Annual Interagency Coordination Report of the Texas HHSC Inspector General and the Attorney General for state fiscal years 2015 and 2016 (6 pp.): coordination activities; key-metrics tables for the HHSC Inspector General (screenings, investigations, referrals, recoveries) and the AG's criminal Medicaid Fraud Control Unit (referrals, cases, convictions, fines and restitution) - both out of scope, they are not the reward programme; the AG's Civil Medicaid Fraud Division table (cases opened, closed, pending, total recovered) for FY2015 and FY2016; 12 matters with recoveries of $2 million or higher named with amounts; ongoing cases (Xerox, dental providers, AstraZeneca) without amounts. The two recovery totals were already recorded. Added the division case counts (6, proposed indicator enforcement_matters), the count of 12 large matters, and as cases the 7 large matters that involve a relator (ex rel. caption or 'relator portions'). Not added as cases: Perrigo ($15,000,000), Breckenridge ($12,500,000), Prasco ($6,950,000), Ranbaxy ($39,750,000) and Glenmark ($25,000,000), which the report lists as Texas investigations or suits with only state and federal portions (no whistleblower). Decision 10 pass (2026-10-10): every named settlement, judgment and newly announced government suit not already in cases.csv added as a case with whistleblower_link=not_stated (the document does not tie it to a whistleblower): 8 cases (5 settlements, 3 pending government suits or claims). Amounts as printed (lower/upper bounds and global vs civil parts explained in notes); individuals only where named by the agency as parties. Added the five matters with only state and federal portions (Perrigo, Breckenridge, Prasco, Ranbaxy, Glenmark), previously left out as 'no whistleblower', as not_stated cases, and the ongoing Xerox, dental-provider and AstraZeneca cases as pending (first appearance; later reports' repeats are not re-recorded).
